Adult social care support is focused on helping you, and those that support you, to be as independent as possible. Our aim is also to enable you to have as much choice as you can about how you live your life.
If you think you're in need of care and support, or the person who cares for you needs support to continue helping you, then you're entitled to have an assessment.
The purpose of the assessment is to work with you to identify what needs you or your carer may have.

My Wellbeing Plan is an easy to use online tool which allows you to create a plan of action to boost your family's wellbeing and make positive changes. Plans can be as long or short as you like and contain anything you want to do from being healthier to getting out and about and meeting new people.
