ELHAP Transition Project
ELHAP Transition Project is for disabled young people, who are moving on from children's services. The ELHAP Transition Project is a springboard, giving young people the opportunities and experiences to decide what they want to do with their lives.
At the end of the Project, young people with be able to make informed decisions about their future including jobs, study, interests, hobbies and friendships. The Transition Project is community based and normally runs on two days of each week.

- Disabled young people (aged 16 – 19 years of age) who are moving on from children’s services. This project is most suitable for those who need support with their transition from childhood to adulthood. Currently, the focus is on young people who do not receive other funding and who would most benefit from exploring work and college opportunities.
