District Nursing Service
The District Nursing service provides holistic nursing assessments and treatments to adults in their own home who have a long term condition or require nursing care following a hospital admission or GP visit. If indicated, following a nursing assessment the service can provide a variety of nursing care in the person's own home. e.g. support with administration of insulin, palliative and end of life care, wound care.
The nursing care provided will be dependent on the health needs identified and treatment required to carry out and support this care. All health needs identified and treatment plans are discussed with the users of the service and where appropriate health promotion and self care will also be included as part of a treatment plan.

Service visits users of the service in their own homes. Wound clinics are available across the borough for minor wounds for mobile service users requiring short term wound care e.g removal of sutures.

Referrals are accepted from all health and social care professionals. Response time is dependent on needs identified at the time the referral is received by the service. All referrals details and requests are via a Single Point of Access number 0844 209 09 09 only.
